5/8/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home of Client to conduct an individual counseling session. Client appeared to be energetic and jovial. QMHP-C informed Client that the focus of the session is learning effective coping and anger management skills to reduce stress when upset or angry. 2. Behavior: Client presented cooperative behaviors throughout session as she was able to discuss challenges with her mother, but she knows how her mother is so she really donât be listening to her or be worried about her. 1.Intervention: QMHP-C began the session by inquiring about Clientâs current thoughts, feelings and emotions. QMHP-C processed with Client as to how she was feeling today. QMHP-C probed Client to talk about the various emotions that she experienced since the last session was conducted with QMHP-C. QMHP-C assisted Client with understanding the cause and effect of emotions. QMHP-C provided Client with role playing activities relevant to various emotions. QMHP-C probed Client to talk about ways that she can utilize her coping skills to assist her. QMHP-C assisted Client with this task by providing alternatives in various settings inclusive of home, school and community. Client and QMHP-C completed a worksheet to begin a series of self-actualization lessons. 2. Intervention: QMHP-C assisted Client with interpreting her answers as well as to the triggers that cause her to loose control of her emotions. QMHP-C concluded with a review of the concepts that were learned while providing Client with support through active listening, role playing, and summarizing what was discussed with her during todayâs session. 1. Response: Client appeared to be receptive to QMHP-Csâ interventions and services that were provided to her during the session. Client was receptive to QMHP-Csâ intervention. She indicated during the session that she was still a little tired but she had energy. Client was cooperative during the role playing activities by providing QMHP-C with feedback to the information that was being presented to her by QMHP-C during todayâs session. Client stated during the session as QMHP-C was reviewing with her different techniques on how to control her angry that she tries to use self-discipline, but sometimes she cannot control her emotions especially when she is already mad. 2. Response: She indicated that when she is really mad she will begin to cry and then she doesnât want to hear anything that anyone has to says (indicating that she is really angry and she may slap someone).
